It is important to contact your health insurance carrier, as well as our office, to confirm participation with your insurance plan. Our front office will verify your insurance coverage before your appointment. At that time it will be confirmed if we participate with your insurance carrier. At the time of service, you will be asked to pay your co-pay, deductible, or co-insurance portion. You will need to bring your photo ID and insurance card to your appointment. Often there is a network symbol on the back of your insurance card that enables you to receive health care services out-of-network. We will work with you and your out-of-network benefits or provide a fee schedule for you to see our physicians on an out-of-network basis. If we are out-of-network with your insurance plan, payment in full is expected at the time of service. We will be happy to provide you with the information necessary for submitting a claim to your insurance carrier.
We also provide medical care to self-paying patients. We will be happy to provide a fee schedule for you to see our physicians on a self-pay basis.
If a referral or authorization is required by your plan, you are responsible for contacting your Primary Care Physician (PCP) to start the process.
